Description
It is used to support smoking cessation in adult smokers (part of a smoking cessation treatment). This type of treatment is called nicotine replacement therapy (NRT).
Dosage for adults (including the elderly)
Nicotine patches are applied once a day.
Treatment is usually started with patches with the highest concentration of 21 mg/24 h and the concentration is reduced according to the following dosing schedule:
- Phase 1: resistance patches 21 mg/24 h - first 6 weeks
- Phase 2: resistance patches 14 mg/24 h - 2 more weeks
- Phase 3: patch concentration 7 mg/24 h - last 2 weeks
- Light smokers (less than 10 cigarettes per day) are advised to start with Phase 2 for 6 weeks and then progress to Phase 3 for a further 2 weeks.
- In heavy smokers (more than 10 cigarettes per day), or if there has been a relapse after nicotine replacement therapy (NRT) or when drug monotherapy is not sufficient to control the urge to smoke), it may also be , beneficial to use more than 1 form of preparation at the same time (eg gum or pill)
Dose adjustment
If significant side effects occur while using the 21 mg/24 hour patch for several days, switch to 14 mg/24 hour patches for 6 weeks, then taper to 7 mg/24 hours for 2 weeks. If adverse effects persist, a doctor should be consulted.
Combined therapy
Patches can be combined with oral forms of nicotine (chewing gum 2 or 4 mg, lozenges 1.5 or 4 mg) . The initial treatment should begin with determining the dose of the patch according to the same rules as for self-administration. About 5 to 6 pieces of oral preparations are recommended daily . Their maximum daily dose when using a concentration of 1.5 mg or 2 mg is 15 pieces, with a concentration of 4 mg 10 pieces. The recommended dose for combination therapy for both light and heavy smokers is given in the tables in SmPC, section 4.2.
Duration of treatment
To achieve optimal treatment results, treatment should last 10 weeks (8 weeks for light smokers or patients who have reduced the strength of the drug). The maximum duration of treatment is 10 weeks. The patient should not continue treatment because chronic nicotine use can be toxic and addictive. As time passes, it is possible to repeat the next treatment in patients who continued to smoke after treatment or quit smoking and started again. If the patient cannot quit smoking within 6 months of treatment with nicotine patches, they should consult a doctor.
Mode of use
Nicotine patches are applied once a day at the same time, preferably in the morning after waking up on a clean and dry area , on skin without hair or an area with little hair. It should not be applied to irritated and reddened skin and areas in skin folds or moving areas (eg over joints). The patch is left on continuously for 24 hours. Then the used patch is removed and a new patch is stuck to a different place on the skin. The patch can be removed at bedtime if necessary (approximately 16 hours after application), but it is recommended to use it for 24 hours to ensure an optimal effect against morning cravings. It should not be left in place for more than 24 hours. If the patch is locked in properly, you can take a short bath, shower or swim with it on. It can be glued in the same place after 7 days at the earliest. During the set time, only 1 patch should be blocked. After handling the patch, always wash your hands with water and avoid contact of the patch with your eyes and nose.
Warning
Smokers should not smoke while quitting. The drug should not be used by non-smokers and occasional smokers. The drug is contraindicated during pregnancy and breastfeeding. It should not be used in children and adolescents. Carryover nicotine dependence may occur. In case of severe renal and/or hepatic dysfunction, the patches should only be used under medical supervision.
Nicotine replacement therapy may worsen symptoms in people who have active esophagitis, inflammation of the mouth or pharynx, gastritis, gastric ulcer, or peptic ulcer. In the event of a severe or persistent local reaction at the application site, patients should discontinue use of the nicotine patch and consult a physician. As part of withdrawal, dizziness may occur (frequency very common), which may adversely affect the ability to drive and use machines. It is necessary to keep patches (even used ones) out of the reach of children, even used patches, because they contain a residual amount of nicotine that is harmful to children. Additional special warnings are given in the SmPC (section 4.4).
